Lead Bank Prepaid Visa Cardholder Agreement
Effective Date: July 21, 2025
This agreement ("Agreement") contains the terms and conditions that apply to the Lead Bank Prepaid Visa (the "Card") issued to you by Lead Bank, a Missouri state-chartered bank (the "Bank"). PLEASE READ THIS AGREEMENT CAREFULLY AND KEEP IT FOR YOUR RECORDS. BY ACTIVATING YOUR CARD, YOU AGREE TO BE BOUND BY THE TERMS AND CONDITIONS OF THIS AGREEMENT. IF YOU DO NOT AGREE TO ALL OF THE TERMS AND CONDITIONS, DO NOT USE YOUR CARD.
blaze.money ("Platform") is operated by Blaze Payments, Inc. ("Developer"). The Card is issued by the Bank pursuant to a license from Visa U.S.A. Inc. and managed by the Developer. The Card may be used everywhere Visa debit cards are accepted. The Card is a prepaid card. The Card is not a credit card or a debit card linked to a deposit account.
In this Agreement, "you," "your," and "yours" mean the individual who is the cardholder. "We," "us," "our," and "Bank" refer to Lead Bank. "Developer" refers to Blaze Payments, Inc.
1. Eligibility
To be eligible for the Card, you must be a resident of the United States, at least 18 years old, and have a valid Social Security Number or Individual Taxpayer Identification Number. You must also have an active account on the Platform. We reserve the right to deny a Card to anyone for any reason not prohibited by law.
2. Identity Verification Disclosure
To help the government fight the funding of terrorism and money laundering activities, federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens a Card account. What this means for you: when you open a Card account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your government-issued photo identification or other identifying documents.
3. Your Card is a Prepaid Card
The Card is a prepaid card. It is not a gift card. It is not a credit card, nor is it a debit card linked to a deposit account. The Card is not designed for recurring or ongoing loading of funds. The Card allows you to spend the value loaded on the Card to make purchases wherever Visa debit cards are accepted. You do not have the ability to add more money to the Card other than as described in this Agreement. The Card has no cash access. The Card is not redeemable for cash except as required by law. The funds on the Card do not earn interest.
The funds on the Card are not insured by the Federal Deposit Insurance Corporation (FDIC).
4. Communications with You
By activating the Card, you agree that we and/or the Developer may contact you by email, telephone (including calls and texts to mobile phones by any automated means, including auto-dialing and pre-recorded messages), or mail at any address or number you provide. You may opt out of promotional communications at any time, but you may not opt out of communications related to your Card or required by law.
5. Your Card Only Holds Fiat Currency
The Card only holds U.S. dollars. The Card does not hold, store, or transact in any cryptocurrency, digital asset, stablecoin, or other virtual currency. If you use cryptocurrency or stablecoins on the Platform, those assets must be converted to U.S. dollars before value is loaded onto your Card. The conversion process is separate from the Card and is not governed by this Agreement.
6. Loading Funds Onto Your Card
How to Load
You may load funds onto the Card through the Platform by converting supported stablecoins (such as USDC) into U.S. dollars. The conversion and loading process is facilitated by the Developer. The maximum amount you may load is subject to limits established by us and/or the Developer from time to time. Loads may be subject to delays for verification or compliance purposes.
Stablecoin Sales Disclaimer
The Developer facilitates the sale of stablecoins in connection with loading value onto the Card. The Bank is not responsible for the sale of stablecoins or any related services. The Bank does not custody, hold, or control any cryptocurrency or stablecoins at any time during the loading process. The conversion of stablecoins to U.S. dollars is not a banking service and is not covered by this Agreement. Any disputes related to the stablecoin sale or conversion should be directed to the Developer at support@blaze.money.
Fees For Value Loading
Please refer to the Fee Schedule included at the end of this Agreement for any fees associated with loading value onto your Card.
Transaction Limitations
The following limitations apply to your Card. These limits may be changed at any time by us or the Developer:
- Maximum Card balance: as set by us and/or the Developer from time to time
- Maximum daily load amount: as set by us and/or the Developer from time to time
- Maximum daily spend amount: as set by us and/or the Developer from time to time
- Maximum single transaction amount: as set by us and/or the Developer from time to time
- Maximum number of transactions per day: as set by us and/or the Developer from time to time
7. Using Your Card
You may use your Card to make purchases at any merchant that accepts Visa debit cards. Each time you use your Card, the amount of the transaction will be deducted from your available balance. You may not use your Card for more than the available balance on the Card. If you attempt a transaction that exceeds your available balance, the transaction will generally be declined. However, if a transaction is processed that exceeds your available balance, you will be responsible for repaying the negative balance immediately upon demand.
Fraud Prevention
We may, in our sole discretion, decline any transaction if we suspect fraud, unauthorized use, or any violation of this Agreement. We may also temporarily suspend your Card pending investigation. We are not liable for any losses or damages resulting from a declined transaction or suspended Card if we acted in good faith.
Other Uses Not Permitted
You may not use your Card for the following:
- ATM withdrawals or cash back
- Gambling or illegal activities
- Purchases of money orders, wire transfers, or similar instruments
- Any activity that violates applicable law or regulation
- Any activity that violates the terms of the Platform or the Developer
Checking Your Balance
You may check your Card balance at any time through the Platform at blaze.money or by contacting customer service at support@blaze.money.
Fees for Use
Please refer to the Fee Schedule included at the end of this Agreement for any fees associated with using your Card.
Stop Payments
You do not have the right to stop payment on any transaction originated by use of your Card. However, if you have set up a recurring payment with a merchant using your Card, and you want to stop the recurring payment, you must contact the merchant directly. If the merchant continues to charge your Card after you have notified the merchant, contact us and we will assist you.
Returns and Refunds
If you return goods or cancel services purchased with your Card, the merchant may process a refund to your Card. Refunds are applied as a credit to your Card balance. The time for refunds to appear varies by merchant. We are not responsible for the timing or delivery of merchant refunds.
Pre-Authorization Holds
Some merchants (such as hotels, gas stations, and rental car companies) may place a temporary hold on your Card for an amount greater than the actual transaction amount. This hold reduces your available balance until the merchant sends us the final transaction amount or the hold expires, typically within 3 to 7 business days. We are not responsible for any losses resulting from pre-authorization holds.
8. Unauthorized Transactions
If you believe your Card has been lost or stolen, or that someone has transferred or may transfer money from your Card without your permission, contact us immediately by calling 1-833-237-8104 or emailing support@blaze.money. You could lose all the money on your Card. If you tell us within 2 business days after you learn of the loss or theft of your Card, you can lose no more than $50 if someone used your Card without your permission. If you do NOT tell us within 2 business days after you learn of the loss or theft of your Card, and we can prove we could have stopped someone from using your Card without your permission if you had told us, you could lose as much as $500.
Also, if your electronic history shows transactions that you did not make, tell us at once. If you do not tell us within 60 days after the earlier of the date you electronically accessed your account (if the unauthorized transaction could be viewed in your electronic history) or the date we sent the FIRST written history on which the unauthorized transaction appeared, you may not get back any money you lost after the 60 days if we can prove that we could have stopped someone from taking the money if you had told us in time. If a good reason (such as a long trip or a hospital stay) kept you from telling us, we will extend the time periods.
9. Error Resolution Procedures
In case of errors or questions about your Card, telephone us at 1-833-237-8104 or write to us at support@blaze.money as soon as you can, if you think an error has occurred on your Card account. We must allow you to report an error until 60 days after the earlier of the date you electronically accessed your account (if the error could be viewed in your electronic history) or the date we sent the FIRST written history on which the error appeared. You may request a written history of your transactions at any time by contacting us. You will need to tell us:
- Your name and Card number;
- Why you believe there is an error, and the dollar amount involved; and
- Approximately when the error took place.
If you tell us orally, we may require that you send us your complaint or question in writing within 10 business days.
We will determine whether an error occurred within 10 business days (20 business days if the transaction occurred within the first 30 days after the Card was activated) after we hear from you and will correct any error promptly. If we need more time, however, we may take up to 45 days (90 days if the transaction occurred within the first 30 days after the Card was activated, or if the transaction was a point-of-sale transaction or a foreign-initiated transaction) to investigate your complaint or question. If we decide to do this, we will credit your Card within 10 business days (20 business days if the transaction occurred within the first 30 days after the Card was activated) for the amount you think is in error, so that you will have access to the money during the time it takes us to complete our investigation. If we ask you to put your complaint or question in writing and we do not receive it within 10 business days, we may not credit your Card.
We will tell you the results within three business days after completing our investigation. If we decide that there was no error, we will send you a written explanation. You may ask for copies of the documents that we used in our investigation.
10. Our Liability for Failures to Make Transfers
If we do not complete a transfer to or from your Card on time or in the correct amount according to our agreement with you, we will be liable for your losses or damages. However, there are some exceptions. We will NOT be liable, for instance:
- If, through no fault of ours, you do not have enough money on your Card to make the transfer;
- If a merchant refuses to accept your Card;
- If an ATM where you are making a transfer does not have enough cash (not applicable if ATM access is not available);
- If the transfer would go over the credit limit on your overdraft line (if applicable);
- If the electronic terminal or system was not working properly and you knew about the breakdown when you started the transfer;
- If circumstances beyond our control (such as fire, flood, or computer or communication failure) prevent the transfer, despite reasonable precautions that we have taken;
- If we have suspended or deactivated your Card for fraud prevention purposes;
- If we have reason to believe the requested transaction is unauthorized;
- Any other exceptions stated in this Agreement or applicable law.
11. Personal Information Changes
It is important that we have your current contact information, including your name, address, phone number, and email address. If your contact information changes, please update it on the Platform or contact us at support@blaze.money. Failure to maintain current contact information may result in your inability to receive important communications about your Card.
12. Securing Access to Your Card
You are responsible for keeping your Card number, PIN (if applicable), and any other security credentials confidential. Do not share your Card number, PIN, or login credentials with anyone. You are responsible for all transactions made with your Card or Card number, whether or not authorized by you, if you have been negligent in safeguarding your Card or Card information. If you believe your Card or Card information has been compromised, contact us immediately.
13. Confidentiality
We will disclose information to third parties about your Card or the transactions you make only (a) where it is necessary for completing transactions; (b) in order to verify the existence and condition of your Card for a third party, such as a merchant; (c) in order to comply with government agency or court orders; (d) as described in our Privacy Policy; or (e) if you give us your written permission.
14. Card Replacements
If your Card is lost, stolen, or damaged, you may request a replacement Card through the Platform or by contacting customer service. A fee may apply for replacement Cards. See the Fee Schedule for details. You will be responsible for all transactions on your old Card until you notify us that it has been lost or stolen.

16. Closing Your Card
You may close your Card at any time by contacting us. If you close your Card, any remaining balance will be returned to you, less any applicable fees, via the method determined by us and/or the Developer. We reserve the right to close your Card at any time for any reason not prohibited by law, including but not limited to suspected fraud, violation of this Agreement, or inactivity. If we close your Card, we will attempt to return any remaining balance to you at the address or via the method we have on file.
17. Telephone Monitoring/Recording
You agree that we and/or the Developer may monitor and/or record telephone calls between you and us and/or the Developer for quality assurance, training, and compliance purposes.
18. Legal Process
If any legal action, such as an attachment, garnishment, levy, or other court order, is brought against your Card, we may refuse to permit (and may be required to refuse to permit) withdrawals or transfers from your Card until the legal action is resolved. We may charge your Card a fee for processing any legal process. See the Fee Schedule for details.
19. Disclaimer
THE CARD IS PROVIDED "AS IS" AND "AS AVAILABLE." TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, WE DISCLAIM ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING, BUT NOT LIMITED TO, IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE CARD WILL BE UNINTERRUPTED, ERROR-FREE, OR COMPLETELY SECURE. WE ARE NOT RESPONSIBLE FOR ANY ACTIONS OR INACTIONS OF ANY MERCHANT, INCLUDING A MERCHANT'S REFUSAL TO ACCEPT YOUR CARD.
20. Amendment and Cancellation
We may amend or change this Agreement at any time. If required by law, we will provide you notice of the change at least 21 days before the change takes effect. Your continued use of the Card after any amendment constitutes your acceptance of the revised Agreement. If you do not agree to any amendment, you must stop using your Card and close it as described in Section 16.
We may also cancel or suspend your Card at any time, with or without cause, in our sole discretion. If we cancel your Card, we will return any remaining balance to you less any applicable fees.
21. Arbitration and Dispute Resolution Agreement
PLEASE READ THIS SECTION CAREFULLY. IT AFFECTS YOUR LEGAL RIGHTS, INCLUDING YOUR RIGHT TO FILE A LAWSUIT IN COURT.
Who Does this Apply to
This Arbitration Agreement applies to you and to us (including the Bank, the Developer, and each of our respective affiliates, successors, assigns, employees, and agents).
What Disputes are Subject
Any dispute, claim, or controversy arising out of or relating to this Agreement, the Card, or the relationship between you and us (including any dispute with the Developer) shall be resolved exclusively by binding arbitration, rather than in court, except that (1) you may assert claims in small claims court if your claims qualify; and (2) either party may seek equitable relief in court for infringement or other misuse of intellectual property rights.
What Rules and Procedures
Arbitration shall be conducted by the American Arbitration Association ("AAA") under its Consumer Arbitration Rules then in effect. The AAA rules are available at www.adr.org or by calling 1-800-778-7879. If the AAA is not available to arbitrate, the parties shall agree on an alternative arbitral forum. The arbitration shall be conducted by a single arbitrator.
Pre-Dispute Resolution
Before initiating arbitration, you agree to first contact us at support@blaze.money and attempt to resolve the dispute informally for at least 30 days. If we cannot resolve the dispute informally, either party may then proceed to arbitration.
Your Right to Reject
You may reject this Arbitration Agreement by sending us written notice within 30 days of first accepting this Agreement. Your notice must include your name, address, Card number, and a clear statement that you wish to reject the Arbitration Agreement. Send your rejection notice to support@blaze.money. If you reject this Arbitration Agreement, it will not affect any other provisions of this Agreement.
Class Action Waiver
YOU AND WE AGREE THAT EACH MAY BRING CLAIMS AGAINST THE OTHER ONLY IN YOUR OR OUR INDIVIDUAL CAPACITY AND NOT AS A PLAINTIFF OR CLASS MEMBER IN ANY PURPORTED CLASS OR REPRESENTATIVE PROCEEDING. The arbitrator may not consolidate more than one person's claims, and may not otherwise preside over any form of a representative or class proceeding. If this specific provision is found to be unenforceable, then the entirety of this Arbitration Agreement shall be null and void.
Jury Waiver
IF FOR ANY REASON A CLAIM PROCEEDS IN COURT RATHER THAN IN ARBITRATION, YOU AND WE EACH WAIVE ANY RIGHT TO A JURY TRIAL.
Severability
If any part of this Arbitration Agreement is found to be invalid or unenforceable, the remaining parts shall remain in full force and effect, except that if the Class Action Waiver is found to be unenforceable, the entire Arbitration Agreement shall be void.
Survivability
This Arbitration Agreement shall survive the termination of this Agreement and your Card.
22. Miscellaneous
Assignment
We may assign this Agreement or any of our rights or obligations under this Agreement without notice to you. You may not assign this Agreement or any of your rights or obligations under this Agreement.
Compliance with Laws
You agree to comply with all applicable federal, state, and local laws and regulations in connection with your use of the Card.
Card Issued under U.S. Law
The Card is issued in the United States and is subject to applicable federal and state laws and regulations.
No Waiver
Our failure to exercise or enforce any right or provision of this Agreement shall not constitute a waiver of such right or provision.
Survival
Sections of this Agreement that by their nature should survive termination shall survive, including without limitation: Unauthorized Transactions, Error Resolution, Arbitration and Dispute Resolution, Disclaimer, and Confidentiality.
Choice of Law & Venue
This Agreement shall be governed by and construed in accordance with federal law and, to the extent not preempted by federal law, the laws of the State of Missouri, without regard to conflict of law principles. Any action or proceeding not subject to arbitration shall be brought in the state or federal courts located in Kansas City, Missouri.
Severability
If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
Entire Agreement
This Agreement, together with the Fee Schedule and any other documents incorporated by reference, constitutes the entire agreement between you and us with respect to the Card and supersedes all prior or contemporaneous understandings, agreements, representations, and warranties.
Business Days
For purposes of this Agreement, "business days" means Monday through Friday, excluding federal holidays.
Privity
This Agreement is between you and the Bank. The Developer is a third-party beneficiary of this Agreement and may enforce certain provisions hereof, but the Developer is not the issuer of your Card. The Bank is solely responsible for the issuance of the Card and the management of the funds loaded onto the Card.
